Our facility operates on an appointment system. Appointments are
available for physical examinations, well child exams, immunizations,
and health assessments, all levels of primary care, referrals, patient
education, family planning, and preventative services. When appropriate,
we refer patients to other departments within the Health & Wellness
Center or out to a specialist in the surrounding community. The Medical
Clinic provides comprehensive out-patient medical care to Tribal
Members, other Native Americans, non-native employees of the
Confederated Tribes of Grand Ronde, and surrounding community members.
However, a new patient must first be seen for evaluation by a provider
before a referral can be made.
To make an appointment with our Medical Clinic, please call during
regular business hours and speak with the medical receptionist. When you
call, please tell the receptionist the specific reason for your visit
so she can make sure to allow enough time for your visit. We realize
that your time is valuable and we try very hard to stay on schedule. In
an effort to maintain a timely schedule, our policy is to reschedule a
patient’s appointment if they show up more than 10 minutes late for
their designated time. We will defer to your physician before
rescheduling. To cancel or change your appointment, please call at least
twenty-four (24) hours in advance. Because of emergencies and medical
complications, there will be times when delays occur and it may even be
necessary to reschedule your appointment.
Our laboratory is a state certified “moderate complexity” lab, which
means that most of our lab work is conducted on-site. We have some of
the same testing capabilities as those you see in the local hospitals.
Radiology is on-site for those patients requiring an x-ray. The goal
of the Lab and X-Ray department is to provide the highest quality
diagnostic and prognostic test and results for the practitioners who
treat our patients. The Lab/X-Ray department does this to aid and
confirm differential diagnosis, to provide criteria for choice of
therapy, and to provide data for ongoing therapy.
When you arrive at the medical clinic, go to the Patient Registration
window (where you check in to see your Provider). From there you will
bring your registration paper over to the Medical Clinic window. At this
window your chart and papers are prepared for your visit to the Medical
Provider. The Medical Clinic receptionist will
note the time of your arrival and appointment time and take your chart
or paperwork back to the appropriate area. The Certified Medical
Assistant will bring you back into the Clinical area to take vital
signs, weight, etc. to prepare you to be seen by your Medical Provider.
If you are coming in for Lab or X-Ray only, you will present to the Lab / X-Ray window and the Lab phlebotomist or
X-Ray technician will bring you back into the Clinical area. At this window,
the Medical Clinic receptionist will ask you to fill out a Health
History Form if you are a new patient, or if you have not been seen in a
long time. These forms are usually updated annually.
The Medical Clinic receptionist also calls and reminds patients of
their medical appointment for the next day. It is very important that we
have your correct telephone number on file in case of any changes in
the Medical Provider’s schedule. If you have any questions, the Medical
