Elder Care Lodges
At our lodges, a wide range of services are available in a comfortable setting where privacy is respected and maximum independence is supported. Each client has a private room with a private bath, phone, and cable TV. There is a whirlpool bath in each lodge. Three home-cooked meals are served daily in our dining room; housekeeping and laundry services are also provided.
We hold recreational and wellness activities planned around our resident's interests. To be eligible for the Lodges you must be a tribal member, tribal spouse or other Native Americans and need assistance with activities of daily living.

Mission Statement
To provide comprehensive, culturally appropriate Behavioral Health services to Native families. The Confederated Tribes of Grand Ronde Behavioral Health Program (CTGR-BHP) is a fully integrated outpatient mental health and alcohol & drug assessment and treatment program licensed by the State of Oregon.
Types of Services
Using a holistic approach, we offer assessments, referral to residential, and outpatient treatment programs, individual, group, family therapy, crisis counseling/intervention, peer support and aftercare services.

Assessment
Assessment is the formal process of evaluating a client’s needs for counseling or referral. Following assessment, treatment of client’s meeting the American Society of Addiction Medicine (ASAM) – II R criteria for outpatient services receive psycho-education on alcohol and drugs at level I for outpatient counseling and education. Mental health clients are eligible for evaluation for psychotherapy, and psychiatric medication management of behavioral disorders.
What to Expect
Initially, clients referred to our program are recorded and processed. Client intake referrals are then discussed by the treatment team and assigned in our weekly staffing meeting. Once assigned, a counselor/therapist contacts the client for an intake session. On average appointments are for 50 minutes, but assessments can take up to two hours.
Once the initial contact, intake, and assessment phase is completed, the client is either referred to specialized treatment or begins a regular schedule of alcohol and drug treatment or mental health counseling. Clients coming into our program for medications only will be assigned to a counselor for an assessment. Once this is completed the client’s case will be discussed with the Psychiatrist and the treatment team to determine the best course of treatment.
Assessment is a formal process that requires several steps:
- A clinical interview
- A psycho-social interview
- Alcohol & drug psychometric testing
- Completion of a treatment plan and/or a formal written report
- Referral to specialized inpatient alcohol and drug treatment or mental health treatment as indicated
The following treatment methods are available and used:
- Level I alcohol & drug treatment
- Mental health counseling sessions are once or twice a week for depression, anxiety, acute and chronic stress
- Dual-diagnosis treatment for co-occurring alcohol/drug and mental health disorders is provided through the treatment team approach
- Psychiatric monitoring and management of medication is provided three times a week
We emphasize a culturally sensitive method that respectfully integrates tribal spirituality and cultural awareness into the full range of behavioral health assessment and treatment.

The Dental Department strives to educate and improve the overall health /dental health of our patients through comprehensive, quality dental services. Our department provides emergency and comprehensive exams, preventive and periodontal services, crown and bridgework, silver and tooth-colored fillings, root canals, extractions, dentures, and any tooth procedures within our scope that would help to improve and/or maintain the health of our patients. Your first new patient exam appointment will include a review of your medical history, thorough soft tissue exam, an oral cancer exam, dental charting of all teeth, and x-rays. Diagnosis and treatment needed will be explained to you at this first visit. Appropriate follow-up visits will be scheduled until needed treatment is complete.
Our team includes three dentists, two hygienists, five dental assistants and dental secretary. We are here to serve you! However, your cooperation is needed in order to provide you with the best care possible. Arriving on time for your appointment is critical to our patient flow. We want to have as much time as possible to give you quality dental care. Good oral hygiene is key to a successful treatment plan. Please brush your teeth prior to coming to your appointment. If you have a fear of the dentist, please let us know in advance so that we may take steps to make your visit more comfortable.
We provide services to all Native Americans. Patients who are not eligible for Contract Health Services are required to pay for any lab services incurred. We strive to see babies as soon as their first tooth erupts. This enables us to provide preventive education to the parents and helps build a positive relationship with the child.
When making an appointment, remember that we schedule four weeks in advance. Once we are booked out four weeks, we ask that patients check back the following week.

Mission Statement
The pharmacy program is committed to providing Tribal members, Native Americans of other tribes, Tribal Employees, and community members with quality medications, useful prescription counseling, and insightful medication chart reviews. By working cooperatively with other medical providers, our goal is to improve a patient’s quality of life by using up-to-date scientific studies, prescription and nonprescription medications, and other complementary therapies.
Getting Started

Prescriptions for Grand Ronde Tribal members and descendants are free of charge, unless the medication is not covered by the Tribal health benefit. Native Americans of other tribes, Non-Native American Tribal employees, and community members could be charged for their medications, depending on insurance coverage. Mission Statement
Purchased Referred Cares mission is to provide funding for Grand Ronde Tribal Members and eligible Descendants; allowing them to receive high quality health care beyond what is available through IHS and Tribal Health facilities.
Who is eligible for P/RC?
- You must be an enrolled Grand Ronde Tribal Member or a direct minor descendant (under age 19) of a P/RC eligible member, residing within the 7-county service delivery area. Those counties are as follows: Marion, Polk, Yamhill, Multnomah, Tillamook, Washington & Clackamas counties. Certain exceptions may apply. Please contact program staff for details.
- A non-Indian woman pregnant with a P/RC eligible member’s child. Rules and restrictions apply. Please contact program staff for details.
- Be enrolled in college or academic institution, vocational program or related training. NOTE: A student must first be eligible for P/RC PRIOR to leaving the service delivery area for school.
- A foster child who is placed in foster care outside of the Grand Ronde service delivery area by court order and who is eligible for P/RC at the time of the court order. Such children continue to be eligible for P/RC while in foster care, or turn 19 years of age.
- Members that are incarcerated are not eligible for P/RC coverage.
- A refusal to apply for alternate resources will result in a denial of payment.
Frequently Asked Questions
WHY DO I HAVE TO APPLY FOR ALTERNATE RESOURCES?
This is required by 42 C.F.R. 136.61, Payer of last resort. Approval of P/RC payment for services is considered after all other Alternate Resources (AR) are applied. Any patient who is potentially eligible is required to apply for or an alternate resource.
WHAT IS AN “ALTERNATE RESOURCE”?
Medicaid, Medicare, Employer Coverage, Private Insurance. Skookum (Shasta) is not insurance.

WHAT SERVICES ARE COVERED BY P/RC?
Most medical services, dental, optical, mental health, chemical dependence and some outside pharmacy and durable medical equipment services.
WHAT IS NOT COVERED BY P/RC?
Non-emergency ER services, chiropractic, massage, naturopaths, personal care items, cosmetic services, luxury or over the counter prescriptions as well as unapproved prescriptions filled at a pharmacy other than Grand Ronde Pharmacy.

The Purchased/Referred Care program is federally funded under P.L.93-638 by Congress for the Grand Ronde Tribe. To receive services through this program, one must meet the Purchased/Referred Care Service eligibility requirements outlined above. All routine medical care, including follow-up visits, must have prior approval.
Some extra benefits for Tribal Members out of the service area, may be available. Please contact Grand Ronde P/RC if you are out of the 7-county service area but need funding assistance for the following services: vision, dental, hearing aid or chemical dependency residential treatment.

What is covered?
Most medical, dental, optical, mental health, chemical dependence, prescriptions and durable medical equipment services. Some services have limited funding, some require pre-authorization and some have patient cost sharing. Please contact Forest County Potawatomi Insurance Department for full benefits.
Who is Eligible?
A. The eligibility requirements for the Confederated Tribes of Grand Ronde membership are as follows. 1. You must be an enrolled member of the Confederated Tribes of Grand Ronde
B. Spouses of enrolled Tribal members are eligible if:
- Both member and spouse are age 55 or older
- Member and spouse have been married for at least 1 year
- Member and spouse reside in the same household
- Members that are incarcerated are not eligible for health coverage

Program Registration and Effective Date
The Skookum Health Assistance Program (SHAP) is a voluntary program and you may enroll at any time by filling out a registration form. The SHAP staff provides you with information about your coverage and a registration form prior to the date you become eligible for coverage. Carefully review the material, complete the registration form, sign (by the member or the parent/guardian), date it, and return it to the SHAP office. Coverage will be decided after receiving the original registration form, signed and dated. The effective date for newborns may vary. Please contact the SHAP or the Forest County Potawatomi Insurance Department for more information
If you are currently enrolled or become eligible for Medicare or any other coverage, please notify the SHAP office immediately. Please send a copy of your card or provide your new coverage information to the SHAP office orForest County Potawatomi Insurance Department.
If you have a change of address, it is important to contact the SHAP office, so your services are not interrupted.
Termination of Coverage
You are no longer eligible for coverage effective the date of the event in which any one of the following events occurs:
1. Your status as an eligible Tribal member changes.
2. You are incarcerated; once released eligibility may be reinstated with the completion of a new registration form.
3. For a non-member spouse, the non-member spouse becomes divorced or legally separated from a Tribal member.
4. For a non-member spouse, monthly payments received 14 days after the 25th of the month will result in termination of coverage for the following month.
5. For a non-member spouse, if the Tribal Member walks on, termination of coverage will end if non-member spouse re-marries.
6. You defraud or attempt to defraud the program.
7. If the program terminates, your coverage terminates the date of the event. If you are hospitalized when any of the above events occurs, coverage will continue until you are discharged.
Eligibility for Alternate Resources
This program is a Payor of Last Resort. If you are eligible for medical benefits under a group health plan, private insurance plan, Medicaid, Medicare, Veterans Administration, Vocational Rehabilitations, Crippled Children’s, homeowners, automobile PIP insurance, or other Federal or State program, you must use these alternate resources first. The Skookum Health Assistance Program is a secondary resource; it is not insurance. It will pay only the eligible medical services that are not covered by any other alternate resources. Enrollment in any and all alternate resources which are available at no cost to you is mandatory. Members who have questions about the coordination of coverage with alternate resources should contact the Skookum Health Assistance Program staff (SHAP) at (800) 775-0095.
